If you are at home
Make a report to an officer in PICA's Investigation and Surveillance Unit. PICA investigates the loss, theft, or damage before the replacement process begins 1.
Submit the completed passport application, PICA's official report, a birth certificate, adoption certificate, or Jamaican citizenship certificate, two identical photos, and valid government photo ID. One photo must be certified by the same official who certifies the application 1.
If you are abroad
Report the loss or theft to local police and contact the nearest Jamaican embassy, high commission, or consulate. Jamaican missions process passport applications and emergency certificates for citizens abroad 3,5.
A lost-passport replacement cannot use PICA's online adult-renewal route. The Ottawa High Commission asks for a police report, citizenship evidence, application, photos, and identity documents. Its listed adult replacement fee is C$173, or C$186 with its processing and mailing charges 4,6. Mission fees vary by country.
Emergency travel document
PICA calls its temporary document an Emergency Certificate. It is issued for specific urgent travel when a regular passport cannot be obtained. PICA tells applicants to confirm that the airline and destination will accept it before applying 2.
In Jamaica, the certificate costs J$2,500 for three-business-day processing or J$5,000 for next-business-day processing. PICA asks for a birth certificate or expired passport, two identical photos, and photo ID if the passport is unavailable 2. Abroad, Jamaican missions normally issue the certificate for one-way travel to Jamaica 3.
Replacement fees and forms
PICA requires the standard passport application form for a lost, stolen, or damaged replacement. Its online renewal option does not accept these cases 1,4.
PICA lists J$6,500 as the adult passport processing fee on its application page, but its lost-passport page directs applicants to a separate replacement-fee route without displaying the replacement amount in the text 1,4. Confirm the full replacement charge with PICA before paying. Mission replacement fees differ.
If you find your passport later
Do not use it. PICA says a passport is no longer valid once reported lost or stolen. Attempted use can cause difficulty or delay at a port of entry 1.
Give the recovered document to PICA or the Jamaican mission handling your case and follow its surrender instructions. Travel only on the replacement passport or accepted emergency document 1,3.
