If you are at home
Report the loss to a passport authority or use the official online route if eligible. The passport becomes invalid from the date of the report 1.
Book the required appointment for the replacement application. Take a compliant biometric photo and an identity document if available. The authority can request extra evidence when identity or citizenship needs confirmation.
If you are abroad
Report immediately at a Polish consular post. You can instead report online with a trusted profile or e-ID, or in writing with a notarised signature 1. Honorary consuls do not accept lost-passport reports 1.
The report itself is free, immediate, and needs no appointment. Book an appointment if you want to apply for a new passport at the same visit 1.
Emergency travel document
After the loss report and identity and citizenship checks, a consul can issue a temporary passport for return to a permanent residence in Poland or another country 1.
For an urgent lost-document case, contact the consular post and describe the situation with supporting records 2. Check that the destination, transit authorities, and airline accept the temporary passport.
Replacement fees and forms
A new adult passport application is made in person at a Polish consular post and booked through e-konsulat 3. Consular fees are set in the mission's current fee table and converted into local currency.
The temporary-passport fee and any outside-hours charge vary by post. Use the schedule of the post handling the case. The loss report itself is free 1,2.
If you find your passport later
Do not use it. A lost passport reported to the consul remains invalid and the report cannot be withdrawn 1. Report the recovery to the competent passport authority in Poland or to a consul abroad and surrender the document as instructed.
